When you first enter our clinic you will immediately be greeted with a smile by our terrific staff. They will share some of the office policies with you and ask you to complete the new patient forms. Or you can print them out from the links below and complete them before you arrive. Once the staff has the completed forms, Dr. Wilner will escort you to the Exam/Consultation room to evaluate your condition. |
